Mass Non-Attendance Form: Please track on this form all students who are:
1) Enrolled in DPS and attended at least part of a school day/period during the 2009-
2010 school year,
2) Were absent without a valid reason during the entire count window (9/24-10/8),
AND
3) Between the ages of 6, on or before 8/1/09, and under 17 as of 10/1/09
